We collect personal information necessary to provide our NDIS services effectively. This may include:

  • Personal Details: Name, address, date of birth, and contact information (e.g., phone number, email).
  • Sensitive Information: Health details, disability status, NDIS plan information, and other data relevant to your care.
  • Other Data: Information from carers, family members, or service providers with your consent, as well as payment or billing details.

We only collect information that is reasonably necessary for our functions and activities, and we aim to obtain it directly from you whenever possible.

2. How We Collect Information

We collect your information through:

  • Direct interactions (e.g., phone calls, emails, or in-person visits to our office at 5c, Level 1/2 Main Street, Point Cook VIC 3030).
  • Forms you complete (e.g., service agreements, contact forms on our website).
  • Third parties, such as the NDIS, healthcare providers, or family members, with your consent or as permitted by law.

3. Why We Use Your Information

We use your personal information to:

  • Deliver and manage NDIS disability support services tailored to your needs.
  • Communicate with you about your care, appointments, or updates.
  • Process payments and comply with NDIS funding requirements.
  • Meet legal, regulatory, and safety obligations under Australian law.
  • Improve our services through internal review and planning (de-identified where possible).

We will not use your information for purposes unrelated to our services without your consent, unless required by law.

  • NDIS authorities or funding bodies to coordinate your support.
  • Healthcare professionals, support workers, or other service providers involved in your care, with your consent.
  • Legal or government entities when required by law (e.g., court orders).

We do not disclose your information overseas unless explicitly authorized by you or necessary for your service delivery, in line with APP 8.

We take reasonable steps to secure your personal information, including:

  • Limiting access to authorized staff trained in privacy obligations.
  • Regularly reviewing and updating our security measures to prevent unauthorized access, loss, or misuse.

6. Accessing and Correcting Your Information

Under the APPs, you have the right to:

  • Request access to the personal information we hold about you.
  • Ask us to correct any inaccurate, incomplete, or outdated details.

7. How Long We Keep Your Information

We retain your information only as long as necessary for the purposes outlined in this policy or as required by law (e.g., NDIS record-keeping obligations). When no longer needed, we securely destroy or de-identify it in accordance with APP 11.

If you have a privacy concern or believe we’ve breached the APPs, please contact us at info@meadowharmonycare.com.au or 0412 868 797. We’ll investigate and respond within 30 days. You may also lodge a complaint with the Office of the Australian Information Commissioner (OAIC) at www.oaic.gov.au.

9. Changes to This Policy

We may update this Privacy Policy from time to time to reflect changes in law or our practices. The latest version will always be available on our website, effective as of the current date: April 2, 2025.
